Temporary online service for application of academic documents

According to types of academic documents, you may download the form and fill the information completely with the attachment of required documents. And submit via email to [email protected] with the scanned documents or clearly taken photos.
The working period may take (1)week and inquire about the process completion to the student affairs department (Ph: 09250073996).
If the processing is completed, go to the student affairs department with the original hard copy document applications and make a payment for application at the finance department.

REQUIRED DOCUMENTS FOR ADMISSION - 2019

You can download the relative of your class registration form on the following.

PROSPECTIVE STUDENT

1. (5) sets of Licensed-sized photo
2. Application Form (Provided from University
3. Original and Copy of Matriculation Marks Certificate
4. Original and Copy of Matriculation Examination Pass Certificate
5. Original and Copies of NRC Card of Applicants and Parents (Recommendation of Immigration and Manpower Department is required to submit if NRC Card is pending/lost or holder is deceased).
6. Physical Health Examination Report from Township Health Department
7. Letter of Undertaking (Provided from University)
8. Recommendation Form from Local Authorities (Attached with University Acceptance Letter)

REMARKS – The admission process will be only regarded as complete registered student after submission of required documents, confirmation of academic office and payment of admission fees at bursary.
